The catch most women never hear: a lab that comes back “normal” is not the same as optimal. Standard reference ranges are wide and built from a general population — including a lot of people who don’t feel their best. We read your results against the levels associated with feeling and functioning well, then line them up with your symptoms.

Fatigue, brain fog, weight changes, low libido, poor sleep, mood swings — these are the symptoms women bring in most often, and they’re also the least specific. Low estradiol can cause them. So can low testosterone, an underactive thyroid, insulin resistance, low vitamin D, or anemia. Several can be true at once, layered on top of each other. The symptom alone doesn’t tell us which lever to pull, and treating the wrong one wastes time, costs money, and can leave the real problem quietly unaddressed while you keep feeling unwell.
Therapy should never start blind
That’s the whole case for testing. Before we recommend bioidentical hormone therapy, a pellet, or any other intervention, we want to see what’s actually happening inside. Testing tells us whether your symptoms map to a hormone deficiency, a thyroid issue, a metabolic pattern, or some combination of the three — and it gives us a clear baseline to measure progress against later, so that “are we improving?” becomes a question we can answer with data instead of a hunch.

Dosing to a number means we give your body what it actually needs — no more, no less — rather than chasing symptoms with a generic prescription that might be too much, too little, or aimed at the wrong target entirely. A meaningful hormone evaluation looks well beyond a single estrogen reading. A comprehensive female panel typically pulls together your sex hormones, your thyroid, and the metabolic markers that quietly shape how you feel. Here’s what those numbers actually mean.
Sex hormones
- Estradiol — the most active estrogen, tied to temperature regulation, sleep, mood, skin, and bone health.
- Progesterone — the calming, sleep-supporting counterpart to estrogen; we look at progesterone for balance.
- Total & free testosterone — total shows how much you make; free shows how much is actually available. Both matter for energy, libido, and strength (testosterone in women).
- FSH & LH — the pituitary signals that help us read where you are in the menopausal transition.
- SHBG — sex hormone binding globulin, the protein that binds hormones; it explains why “total” and “free” can tell very different stories.
A full thyroid panel
Thyroid symptoms mimic hormone symptoms almost perfectly, so we test it properly: TSH, free T3, free T4, and thyroid antibodies — not just a lone TSH. That fuller view is the difference between catching a real thyroid pattern and missing it; learn more about thyroid optimization.
Metabolic & general markers
- Fasting glucose, insulin & A1c — how your body handles blood sugar; insulin resistance drives weight gain and fatigue and is central to PCOS.
- Lipid panel — cholesterol and triglycerides, which shift with hormone status.
- Vitamin D — low levels are common and quietly affect mood, energy, and bone health.
- CBC — a complete blood count to rule out anemia and other causes of fatigue.
“In range” and “feeling well” are not the same thing.
This is the single most important idea in hormone testing, and the one women are most surprised to hear. A result flagged “normal” on a standard lab report only means it falls inside a wide statistical band — a range built from a large, mixed population that includes plenty of people who are tired, struggling, and decidedly not thriving.
Where reference ranges come from
Standard reference ranges describe what’s common, not what’s ideal. They’re also often broad enough to span decades of age and very different stages of life, lumping a thriving 30-year-old in with someone three decades older. Because they’re so wide, you can sit comfortably “in range” while running near the very bottom of it — technically fine on paper, genuinely unwell in your body. That gap is exactly why so many women are told their labs are normal while they feel anything but, and why a printout that says “no action needed” can be so frustrating.
Reading for optimal
We interpret your numbers against the ranges associated with feeling and functioning at your best, then weigh them in the context of your age, your goals, and how you actually feel. Optimal isn’t a single magic number — it’s the level where your symptoms resolve and your health markers line up. That nuance is the heart of individualized, lab-driven care.

Numbers never get the final word on their own. We line your results up against your symptoms — the sleep, the energy, the mood, the libido, the way your body composition has shifted — because a value that’s “fine” but paired with classic deficiency symptoms still tells a story. Blood testing
A serum blood draw is the workhorse. It’s precise, widely validated, and the standard for measuring estradiol, testosterone (total and free), thyroid, and your metabolic and general markers. For most women, a well-chosen blood panel answers the questions that matter, is easy to repeat over time, and gives us a clean baseline to monitor against as your plan unfolds.

Matching the plan to the picture
What that plan looks like depends entirely on what your results show. It might mean bioidentical hormone therapy to restore estrogen, progesterone, or testosterone; the steady delivery of pellet therapy; support through menopause; a focus on PCOS; or attention to your thyroid. Many plans address more than one of these together.

A single set of labs is a snapshot in time, and your hormones don’t hold still. Perimenopause shifts month to month, treatment changes your numbers on purpose, and life — stress, sleep, weight, age — keeps moving the picture. That’s why testing is a process, not a one-time event.

We establish a baseline before any therapy, then re-test after you’ve been on a plan to confirm we’ve reached optimal levels — not just changed them. That follow-up is how we know the plan is actually working rather than assuming it is.
How often we re-check
Timing depends on your plan and how you feel. Early on, we re-test sooner to dial in your dose and confirm your body is responding the way we’d expect; once you’re stable and feeling well, we move to a steadier monitoring rhythm. Throughout, we track both your labs and your symptoms, because the two together — not either one alone — are what tell us whether you’re truly optimized or simply somewhere in the range.
